Step 2: What kinds of information do I provide to the potential funding source?

The following are some suggestions for items and/or services needed as you prepare for providing information about the AT device to a funding provider.

DOCUMENTATION:
Once the individual’s need has been defined and documented, the AT devices and services required for this need must be identified and described. This includes a specific written justification from appropriate professionals. Supportive materials that are often used in documenting needs include:

  • Physician's prescription for the AT devices or services and a letter of medical necessity.
  • Letters of medical necessity from the other licensed health care professionals such as your physical therapist, occupational therapist, speech therapist, etc.
  • Letter of medical diagnosis that may further provide specific medical information and needs.
  • Explanation of the individual's functional skills and capabilities without the AT device and how those skills and capabilities will improve with the requested AT.
  • Photograph or video of the individual. (This may help to demonstrate the need.)
  • Literature concerning the specific AT device, with specifications as they apply to this individual. These specifications should include cost and features.
  • All of this information is used to develop a written "documentation of need" to present to the funding source.

COST:
At this point in the funding process, you should identify vendors from whom the AT can be purchased and find out how much the AT will cost. The funding source will need to know the cost, and the vendor may be able to help you in the funding process.

When reviewing your request, all funding sources will determine if the charge for the AT device or service is reasonable. Consider some of the following questions:

  • Is there a way to fabricate this device or equipment?
  • Can it be borrowed from an "equipment loan closet"?
  • Are the benefits of the AT device outweigh the expense?
  • Is there a less expensive device that can meet your needs just as effectively?

You may need to determine if there are alternative devices that would also meet your needs. Research these alternatives before you apply to a funding source. If you find that there are no alternative devices to meet your particular needs, be sure to state that in all your documentation.

Do not let the cost of the AT keep you from selecting the most appropriate device for your needs.
