Assistive Technology Consideration Form

This Assistive Technology (AT) Consideration Form is an instrument developed by the team at Georgia Tools for Life (TFL) and the Center for Inclusive Design and Innovation at Georgia Tech (CIDI). The AT Consideration Form is designed to assist Assistive Technology practitioners and others, including parents, teachers, and caregivers, in selecting appropriate AT items for use by a person with a disability. 

The AT Consideration Form is designed to help individuals gather the necessary information and ask the right questions in order to make choices and decisions about AT based on evidence and best practices. The AT Consideration Form is based upon the HAAT (Human, Activity, Assistive Technology) model for choosing AT and the SETT (Student, Environment, Task, Technology) framework for AT decision making. The instrument provides an effective basis for periodic screenings to Identify individual AT Needs and for justifying AT requests. The form is a downloadable, fillable PDF form. It is available for use by permission through GA Tools for Life website. 

With so many individuals desiring to age in place and people with disabilities looking to live independently in their local communities, effective use of AT tools has become especially valuable. As more family and friends step up to provide support for these individuals, this form can be very useful as the need for more information with which to make decisions becomes evident. 

The AT Consideration Form provides a systematic way to:

· Consider a person’s individual needs during or within a specified daily activity
· Identify additional factors that may impact client needs, before or after the activity
· Determine potential AT tools, devices and services to increase or maintain independence
· Serve as a record of AT previously considered and why
